 rivate, non-profit 501 (c)(3) community health center, that has been servin
 g low-income, uninsured, underinsured, and underserved populations in Centr
 al Florida since 1977. We operate nine service delivery locations within Or
 ange and Seminole Counties, which includes ten neighborhood health centers.
  We provide services in areas throughout Central Florida, which are Health 
 Resources and Services Administration (HRSA) designated as medically unders
 erved populations and health professional shortage areas. We are recognized
  as a patient-centered medical home with full accreditation by Accreditatio
 n Association of Ambulatory Health Care, Inc. (AAAHC). As a Federally Quali
 fied Health Center, we offer a sliding fee scale to those that are uninsure
 d, we accept Medicare and Medicaid, and over 25 major health and HMO insura
 nce plans.
