 h is a private, non-profit 501 (c)(3) community health center, that has bee
 n serving low-income, uninsured, underinsured, and underserved populations 
 in Central Florida since 1977. We operate nine service delivery locations w
 ithin Orange and Seminole Counties, which includes ten neighborhood health 
 centers. We provide services in areas throughout Central Florida, which are
  Health Resources and Services Administration (HRSA) designated as medicall
 y underserved populations and health professional shortage areas. We are re
 cognized as a patient-centered medical home with full accreditation by Accr
 editation Association of Ambulatory Health Care, Inc. (AAAHC). As a Federal
 ly Qualified Health Center, we offer a sliding fee scale to those that are 
 uninsured, we accept Medicare and Medicaid, and over 25 major health and HM
 O insurance plans.
