Tennis Summer Camps 2024
Registration form for all tennis camps hosted by SM Swiss Tennis Academy. All equipment will be provided. Every camp will divide children into groups based on age and skill levels. Drop off time is 15 minutes before camp starts, pick up time no longer than 15 minutes after camp ends. All camps have limited spots available.

I herby acknowledge that there is a risk of injury from participating in the tennis program and its related activities. I HERBY WAVE AND RELEASE SM Swiss Tennis Academy, Stefanie Majstorovic, and its employees from any liability claims for damage. PAYMENTS: Payment is due by May 15th, 2024. Cancellations made on or before May 31st will receive a full refund minus $25 processing fee. Cancellations made after May 31st but more than 2 weeks prior to the start of the camp will receive a 50% refund. No refunds or discounts will be given for cancellations made less
 than 2 weeks prior to the start of a camp or for missing camp due to sickness or vacation. In a medical emergency I understand that every effort will be made to contact me, the child’s parent or guardian. If I cannot be reached, I herby give my permission to the attending physician to administer emergency care to my child pending my arrival at the medical facility.
